Clinical care for Dr. Rastinehad's patients is provided through Northwell Health. The full Notice of Privacy Practices that governs your medical record is maintained by Northwell Health and is provided to you at your first visit.

This website is informational and is not a patient portal. Forms on this site (including the contact form) are not encrypted patient messaging and should not be used to transmit protected health information such as medical record numbers, lab values, imaging details, or pathology reports.

How to share PHI securely

• Use Northwell MyChart to message your care team and share records securely.
• Call the office at (212) 434-6580 for the secure intake address used to ship imaging discs or upload images.

Your rights under HIPAA

You have the right to inspect and copy your medical record, request amendments, receive an accounting of disclosures, request restrictions or alternative communications, and obtain a paper copy of the full Northwell Notice of Privacy Practices. Detailed instructions are available on the Northwell Health website.

To file a privacy complaint, you may contact the Northwell Health Privacy Office or the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
